I believe it is real. I saw one with his head out of a car window that was close to this size. I also have a St Bernard very close to this size.

 

The world record for the tallest dog is just over 7' tall when standing on it's hind legs.

Even Mastiff would have to be extremely over weight to get that big. The dog in the pic doesn't look to be excessively over weight. I think the head has probalby been photo shopped.

That's huge. My search yielded "Moose", the 291 lb Mastiff. I guess I should've searched a little harder.
