In the Alabama/Michigan State game right before half when MSU had the ball was a great example of man and zone. One play they throw to the wr over the CB with his back to the Qb. The next play the Qb thought it was the same coverage because the CB gave the same pre snap alignment but it was soft Cover 2/Read and the CB was facing the Qb and made an easy pick. Qb did not scan the whole field to see both safeties alignment on his pre snap read, he made an assumption.....

Exactly, based on their personnel they might just walk out on your slots or #2 receivers and try to get hands on to prevent them from releasing up field and play Cover 3. Or they walk out one lb on your #2 and the other falls back to safety and then they play a lot of games with their coverages. But remember who ever has the marker at the dry erase board wins!

Almost all defenses the Will is the most difficult to account for, a lot of the times they are their teams leading tackler.

NF Football Rules Changes - pre-1981 - Football.Refs.Org Here is all the rule changes since forever. The site has them divided up in about 5 year increments. I do not see anywhere that the rule was changed, meaning it has always been the rule. Maybe, I overlooked it?
