Mayfield in the past 43 seasons starting in 1976:

 

Final Four appearance in 36 of 43 years (84%)

State Finals appearance in 23 of 43 years (53%)

State Champion 12 times in 43 years (28%)

Belfry won 4 straight 13, 14, 15, and 16

Mayfield and Highlands are way above the rest. I'd give the edge to Highlands. And I'm a Beechwood fan. Birds were just insanely good at a high level. Just murdered everyone for 10+ years.
