Assuming the dugout is located in a relatively normal spot (middle of the baseline), it would be a foul ball.

Here's one:

 

#1) Bottom 9th, 1 out, men 2nd and 3rd base, 2-2 score. Pitcher decides to intentionally walk hitter. Catcher stands up to signal intentional walk, then steps outside catcher's box, and pitcher then throws pitch to catcher, who quickly shoots down to third base and catches sleeping runner off bag. Call?

 

#2) Same setup, but this time they intend to pitch to hitter (don't ask me why, they just do!). Before pitcher throws ball, catcher again pops out of catcher's box, receives pitch and picks runner off third. Call?

 

I could do this stuff all day. I can't count how many times I've sat in a training room class making up questions to give to another person during the break and then answering his questions next time class convened. I think it's a disease or something.

Well I checked my MLB rulebook and see I butchered a couple answers, but I won't say which ones. At least it proves I did it on my own. Of course, it also proves I'm not exactly a baseball genius.

Assuming that the catcher stepped completely outside of the catcher's box BEFORE the pitcher started his motion and its during an intentional walk, its a balk on the pitcher.

 

The "catcher's balk" is a misnomer. Its still a balk on the pitcher. However in #2 since it was not during an intentional walk, its a legal play by the catcher.

 

On the "home to 2nd" distance, if you know your Pythagorean Theorem, you can figure it out.
