My Dr. office left a message stating that there is a new, 10 page form that I need to download from a website for his eligibility physical. They didn't have the website info. No one at the school is answering the phones. Anyone know anything about this??? Help is appreciated!

Just got a call from one of the coaches...there IS a new form! And, at least for the Highlands players, they can pick up a copy at the main office of the school. If anyone finds out the wesite locale, please post it so others may have it!

The PDF is 10 pages long; but you have to scroll down to page 7 to get to the actual form that has to be filled out. The first 6 pages are just all the rules and by-laws.

See Post #4 or Visit the KHSAA website and click on "Forms", than General and on the right, will be GE4 - Consent.

Hey now we have a rule about posting false information on this site... :D
I'm just excited, to see the word "speed" and my name, in the same sentence.

 

I used to get timed, by an hour glass....:D
