Does anyone know what the difference is, our school already did the school physicals and this form seems to look identical...

I know, that all 10 pages, must be distributed to "each" student-athlete.

The previous form has gone from a single page landscape print, to a two page standard print, but the info is the same. The rest of the 10 pages is extra stuff that the parents need, but as far as I know, need not return to the school.
