May 31, 201511 yr If I am waiving all liability for KHSAA so that my child may participate in high school athletics, what's with some of these questions and "physician reminders"?? What is the legitimate medical reason for them to inquire how old a girl is/was when she started her period and how many periods she has had in the past 12 months? They provide a list of "sensitive issues" for doctors to inquire on - including condom usage? What are they doing with this information???

May 31, 201511 yr Author There are also questions about being on a special diet or avoiding certain types of food? If my kid is a vegan, vegetarian or is avoiding gluten - why do they care?? I am way bothered by this and feel it is unnecessarily intrusive.
May 31, 201511 yr Its all insurance liability, you can thank the parents that have sued over the years for this and that.

May 31, 201511 yr FYI, I had a daughter play HS sports in Ohio from 2004-2008 and the forms from the OHSAA had NO questions like that. They seem way out of line to me...
June 1, 201511 yr Author I think it's out of line too. I'm not buying the dietary restrictions answer either. I think the form should begin and end with "Is this minor fit to play?" It is my understanding that the school keeps one copy of the form and the other is forwarded to the state. Anyone know if that is correct?? http://khsaa.org/forms/CombinedGE04andPPEForm.pdf
June 1, 201511 yr Its all insurance liability, you can thank the parents that have sued over the years for this and that. This. Since the KHSAA (and if I'm not mistaken the National Federation) mandate a physical prior to sports participation they could be held legally liable if student has a underlying medical issue that should have been found on a physical, this includes gynecological issues for girls. It's going to get worse. They are not far from mandating some kind of baseline neurological testing for for every student athlete and there has even been talk of the NF recommending a 1 time echo-cardiogram for all student athletes. I know that physicians that sign off on the exam have also demanded a more thorough health history questionnaire. They are ultimately the ones who put their names on the line when they sign off on a physical and I don't blame them. Before they sign that a student is healthy enough to play a sport they need to have good information. The question about condoms is simply a suggestion and not a necessity. A pre-participation physical shouldn't just be a rubber stamp.

I wanna know where my kid's information winds up. My boys tell me that before a road trip, Coach says "Remind me again, guys, who needs something different to eat??" and that no one is shy about saying no chicken for me or no potatoes or bread for me or whatever. The form states that you are allowing them to use and share this information. For what? With whom? And I know for a fact our doctor in no way copies the form or puts all those answers into his records...he isn't protected as to what he has signed off on.

This portion states what the information can be shared for: The student and parent/legal guardian, individually and on behalf of this student, consent to the high school and the KHSAA and their representatives to use and disclose the necessary personally identifiable information from the student’s education records including academic, financial and health care information, to third parties including school representatives, coaches, athletic trainers, medical facilities, medical staffs, KHSAA legal counsel and the media, for the purpose of receiving proper/necessary medical care and complying with the KHSAA bylaws, including making determinations regarding eligibility to participate in interscholastic athletics and any administrative or legal proceedings resulting from participation or attempted participation in interscholastic athletics, without such disclosure constituting a violation of rights under the Family Educational Rights and Privacy Act. The student and parent/legal guardian, individually and on behalf of this student, further release the high school, the KHSAA and their representatives from any and all claims arising out of the use and disclosure of said necessary personally identifiable information, and agree to release to the high school, the KHSAA, and their representatives, upon request, the detailed and completed application for financial aid. The schools are responsible for storing the forms they are not sent to KHSAA. It becomes part of the student's record at the school. The days of stopping by the doctor for a quick stethoscope to the chest and turn your head a cough then a quick note that XYZ is OK for sports is over and that's a good thing as far as I'm concerned.
