Someone referred to the first part of this rule on throw-ins in a prior post, but the full unwritten rule, I think, goes as follows:

#6. When the center and AR are properly positioned, but do not see who last touched the ball when it exited the touchline:

#6a. Except as provided in 6c and 6d, the throw-in is awarded to the team that first first reacts like the throw belongs to them,

#6b. Thereafter, the official alternates the award of uncertain throws.

#6c. If it's in the final third of the field, the throw-in is awarded to the defensive team,

#6d. When, in the opinion of the referee, one team has constantly questioned the referee's calls, the referee, at his or her discretion, may award the throw-in to the other team.

#6e. Each time rule #6 is applied in a game, there will be at least a dozen fans of the team not awarded the throw-in who clearly saw the ball comes off the other team,

#6f. Notwithstanding rule #6c, if a goal is scored from the run of play immediately following throw-in, multiple posts to BGP are warranted.

#6g. If the center and AR were out of position, one of the posts pursuant to #6f is required to be from OTB84.

Saw in the US vs Germany World Cup game when Clint Dempsey was hurt a German kicked the ball out of bounds during the game to allow him to rest. After Dempsey was removed from the field, USA kicks the ball back to Germany.

I believe this happened in the Boone v Simon Kenton game. SK player was down and another SK player kicked it out. Boone pooched it to SK on the subsequent throw-in.

It is done as a courtesy. The referee has the option to stop play if he/she deems a player is seriously injured. Otherwise, the ref. will allow play to continue if the down player's team is not in possesion of the ball. For this reason, the team with the ball, if they see the down player, will go ahead and play the ball out of bounds to stop play. Then the courtesy is for the team with the down player to throw it in back to the team that kicked it out. There are times they will not extend this courtesy....when they believe the down player is faking an injury to stop an advantage, when they don't see the down player, or when they are inside the 18 ready to score. I have never seen a team give up a shot to kick the ball out.
