Is there such a thing as a regular season district title?

Back when I played, if you went undefeated in regular season district game, you won’t be district and the # 1 seed.

Maybe it change, and you just get #1 seed.

Yeah in football you are considered district champ if you win the district in the regular season. In basketball it's just for seeding. Teams recognize winning the district tourney as the district champ and get the trophy to show it, and usually a date on their banner.
