If the receiver/runner fakes like he has the ball, and that fake is effective enough so that the defender tackles him, how can that be called defensive holding?

Behind the LOS its not a hold. Defense can tackle anyone in the backfield. Once the runner is across the LOS and in the pattern its a hold.

 

I believe....

If it's an eligible receiver on a pass play, it's holding.
