Yes all in the bottom of the 8th inning with 2 outs and the 0-2 count, the HBP, the first single, (at work so don't have my book) but if I remember correctly a walk, then the ground ball to (4) that should have either been a (4-6 fc) or (4-3) to end the game.

Correct on the sequence. It was HBP and single down the third baseline. On the single the runner on first went to third which drew a throw allowing the hitter to advance to second. So second and third 2 outs. The 3 hitter was then intentionally walked to set up a force anywhere. Then came the E4 which ended it.
